UP AYUSH Counselling 2026: BHMS Document Checklist (Print)

Vaibhav Kumar rai·

If you have cleared NEET and are aiming for a BHMS seat through UP AYUSH counselling, the one thing that derails more candidates than a low rank is a missing document on verification day. Every year, nodal centres across Lucknow, Kanpur, Prayagraj and Gorakhpur turn away candidates who had the marks but forgot a single certificate – a gap year affidavit, an income certificate that had expired, a migration certificate still stuck at their old school. This checklist is built around what UP AYUSH counselling (upayushcounseling.upsdc.gov.in) typically asks for, year after year. Treat it as your packing list, but always cross-check the exact requirements against the official information bulletin released before your counselling round, since small details do change from year to year.

Why documents matter as much as your NEET rank

UP AYUSH counselling for BHMS, BAMS and BUMS seats runs in online rounds, but admission isn’t final until physical document verification at a designated nodal centre. Your seat allotment letter only holds if your documents check out. A discrepancy – say, your name spelled differently on your Aadhaar card versus your Class 10 marksheet – can hold up your admission while everyone else in the queue moves ahead of you. Carrying both originals and photocopies, in the right order, in a single folder, saves you that stress.

The core checklist: documents every BHMS candidate needs

Keep at least two sets of self-attested photocopies of each document along with the original. Most centres keep one set and hand your originals back after verification, but rules vary by round and centre.

  • NEET UG admit card (the year you qualified)
  • NEET UG result/rank letter showing your All India Rank and score
  • Class 10 marksheet and certificate – for date of birth proof
  • Class 12 marksheet and certificate – with Physics, Chemistry, Biology marks visible
  • Provisional/migration certificate from your Class 12 board, if applicable
  • Transfer certificate (TC) from your last attended school
  • Character certificate from your last school or college
  • Domicile/residence certificate of Uttar Pradesh – required if you’re applying under the UP state quota
  • Category certificate (SC/ST/OBC-NCL/EWS) issued by a competent authority, if claiming reservation
  • Income certificate – mandatory for EWS candidates, usually needs to be current within the financial year
  • Aadhaar card and one more government photo ID (PAN, passport, voter ID)
  • Passport-size photographs – carry at least 8-10 in the same format used during NEET registration
  • Gap year affidavit (notarised) if there’s a break between Class 12 and your NEET attempt
  • Medical fitness certificate from a registered medical practitioner
  • Counselling registration fee receipt and seat allotment letter printout
  • Anti-ragging undertaking, filled and signed on the official anti-ragging portal (this is generated online and printed)

What if you don’t have a domicile certificate yet?

This trips up a lot of students from outside UP who got allotted a seat under the all-India quota, and also UP residents who simply never applied for one. If you’re from UP and applying under the state quota, start the domicile certificate application on the e-Sathi or tehsil portal weeks before counselling begins – it can take 7 to 15 days depending on your district. If you’re from another state and allotted a seat under the all-India quota, a domicile certificate generally isn’t required for that quota, but keep your permanent address proof (Aadhaar, voter ID, ration card) ready since centres sometimes ask for it anyway.

Category and EWS certificates: check the validity window

OBC-NCL and EWS certificates usually need to be issued within the current financial year – an EWS certificate from last year’s cycle may be rejected even if nothing about your family’s status has changed. If your certificate is more than a few months old, it’s worth visiting your tehsil office before counselling starts to get a fresh one, rather than discovering the problem at the verification desk.

Gap year? Don’t skip the affidavit

If you took a year (or more) after Class 12 to prepare for NEET, most AYUSH counselling authorities ask for a notarised gap year affidavit explaining what you did during that period – typically that you were preparing for competitive exams and were not employed. This is a one-page document, easy to get notarised at any local notary, but candidates often forget it entirely because school-level admissions rarely ask for it.

Organising your folder for verification day

A simple way that works well: use a transparent folder with separate plastic sleeves, arranged in the order centres usually check – identity and date of birth proof first, then academic certificates in chronological order (10th, 12th, TC, migration), then NEET documents, then category/domicile/income certificates, and photographs and the anti-ragging undertaking last. Label each sleeve with a sticky note. It looks basic, but verification staff handling hundreds of candidates a day notice – and appreciate – a folder that’s easy to flip through, and it keeps you from fumbling when your turn comes.

A note on choice filling and reporting

Document readiness isn’t just for the final reporting day – some rounds require you to upload scanned copies during registration itself, before choice filling opens. Scan everything in advance, in the file size and format specified (usually PDF or JPEG under a certain size limit), so you’re not scrambling to find a scanner on the last day of a round. Frequently asked questions

Is the domicile certificate compulsory for all BHMS applicants in UP?

It’s typically required only for candidates applying under the Uttar Pradesh state quota. All-India quota candidates from other states usually don’t need it for that quota, but always confirm against the current year’s information bulletin, since requirements can be updated.

What happens if I’m missing one document on verification day?

Policies vary by round and centre – some allow a short window to submit a pending document, others may mark your verification incomplete. It’s far safer to call your nodal centre in advance or check the official portal’s FAQ section than to risk it on the day.

Do I need notarised copies of all certificates, or just self-attested ones?

Most academic certificates only need self-attestation (sign across a photocopy stating it’s a true copy). Notarisation is usually required only for affidavits, such as the gap year affidavit or any name-change declaration.

Can I carry digital copies on my phone instead of printouts?

No – verification is done on physical originals and photocopies. Always carry printed copies; a phone screen is not accepted as a substitute during document checks.

Where can I confirm the exact, current-year document list?

The Department of AYUSH, Uttar Pradesh publishes a detailed information bulletin before each counselling cycle on upayushcounseling.upsdc.gov.in. Read it fully before your verification appointment, as document requirements are revised most years.

Once you’re through verification and have your seat confirmed, you can also look up your new college on Homoeopaths.org’s search to connect with seniors and alumni from the same institution – a quick way to get answers to the dozens of small questions that come up in your first week.
